Low Back Pain Caused at Work

Mark, a 45 year old male, stated that while at work yesterday bending over to lift a chain off a dock plate, he experienced the immediate onset of sharp low back pain. Mark continued to work but progressively became stiffer along with continued pain throughout the day. This patient further explains within several hours experiencing radiation of pain into his left buttocks and back of his leg.

Examination revealed a noticeable loss of range of motion stemming from the patients low back and associated muscle spasms. Further orthopedic examination revealed nerve impingement coming from the lower lumbar spine producing sciatica down his left leg. This patient was briefed on the topic that although most sciatic problems respond well with manipulation and associated conservative care, some may be the result of a disc herniation. These can be more resistant to conservative care. Patient was instructed that if we did not see a reasonable response to therapy within 2-3 weeks we would most likely proceed with an MRI at his low back and if necessary refer out to an orthopedist.

Therapy was initiated consisting of manipulation to restore range of motion, along with electric-stim to reduce the muscle spasms. The patient was able to return to work on light duty within 2 days and within 1 week patient evaluated his pain 60-70% less than when he first started therapy. Patient was returned to regular duty 1 week after starting therapy. Within 2 weeks Mark reported of no radiation of pain into his left buttocks, and leg, and experiencing only some low back stiffness/achiness’ for approximately 1 hour when first getting up in the morning. This patient was followed up once more in 3 weeks with essentially no symptomatology and exhibiting normal orthopedic testing. Mark was instructed on core exercises to prevent re-occurrence of similar problems and released from care.
